Employee Insurance Coverage Notices
- Form 1 - For employers who offer a health plan to some or all employees.
- Form 2 - For employers who do not offer a health plan.
The purpose of the Neighborhood Assistance Program (NAP) is to encourage businesses, trusts, and individuals to give to approved 501c3 organizations for the benefit of underserved communities. In return for these contributions, tax credits equal to 65 percent of the donation may be applied against state income tax liability.
